Lumbar disc herniation is a common disease that troubles modern people. When the acute pain is unbearable, many patients adopt the strategy of "absolute bed rest" and even believe that "the longer they lie down, the faster the recovery". But long-term bed rest may backfire! The following are common misconceptions and scientific suggestions about bed rest to help patients avoid detours。

Five major misconceptions

Misconception 1

Must the lumbar protrusion be absolutely bedridden and not allowed to descend to the ground?


Truth: In the acute phase, short-term bed rest is required, but complete immobilization is not necessary!

Short term bed rest (1-2 weeks): During the acute phase of severe pain, bed rest can reduce intervertebral disc pressure and alleviate nerve root edema. However, it should be noted that 'bed rest' does not mean staying still for 24 hours. You can get out of bed to use the toilet, eat, and avoid prolonged sitting or bending over.

Early activity: Studies have shown that there is no significant difference in long-term efficacy between bed rest and normal activity. After the symptoms are relieved, daily activities should be gradually resumed to prevent muscle atrophy and lumbar stiffness 。

Is a hard bed better?


Truth: Medium hardness mattresses are more in line with physiological curvature!

Hardboard bed ≠ Sleeping directly on wooden boards: A hard mattress can cause the waist to be suspended and muscles to tense; A mattress that is too soft cannot support the lumbar spine. The ideal mattress should follow the "3:1 principle" (such as sinking 1cm with a thickness of 3cm) and fit the curves of the shoulders, waist, and buttocks.

Correct sleeping position:

Lie flat: Place 1-2 pillows under your knees to reduce pressure on your waist.

Lying on the side: with a pillow between your legs, keep your lumbar spine in a neutral position 


Misconception 3

The longer the bed rest time, the faster the recovery?


Truth: Staying in bed for more than 3 weeks may worsen the condition!

Muscle atrophy: Long term bed rest leads to lower back muscle weakness, decreased lumbar stability, and increased risk of recurrence 。

Scientific time: In the acute phase, bed rest should not exceed 2 weeks, and in the chronic phase, moderate activity should be based on the principle of "not causing pain". There was no significant difference in therapeutic efficacy. After the symptoms are relieved, daily activities should be gradually resumed to prevent muscle atrophy and lumbar stiffness。


Misconception 4

Not moving at all during bed rest?


Truth: Gentle rehabilitation training can begin in the acute phase!

Bed exercise:

Straight leg elevation: prevent nerve root adhesion, 3 groups per day, 10 times per group 。

abdominal breathing:

Enhance abdominal muscle strength and reduce lumbar load 。

Get out of bed activity: Wear waist protection to avoid bending over and lifting heavy objects, gradually transition from standing for a short time to walking


Misconception 5

Does waist protection need to be worn for a long time?


Truth: Waist protection is a "temporary crutch", long-term dependence can lead to muscle weakness!

Usage scenario: It is only recommended to wear it during the acute phase or when carrying heavy loads, and should not be worn for more than 4 hours per day 。

Alternative solution: Strengthen the core muscle group through methods such as "bridge exercises" and "flat support" to gradually eliminate dependence on protective gear。
